The village of Khmelyska on the Samets River is located 15 kilometers west of Pidvolochysk, 5 kilometers from the M-12 highway "Ternopil - Khmelnytskyi".
Known since 1565 as Khmelyshcha. The name probably comes from the hop thickets that once covered the ravines around the village.
In 1807, the first stone church of the Immaculate Conception of the Most Holy Theotokos was built. The first Catholic chapel was founded at the expense of the Polish community of the village in 1899. The land for construction was allocated by the landowner Rosenstock. The current Ascension Church was built in 1931-38.

Село Хмелиська на річці Самець розташоване в 15 кілометрах на захід від Підволочиська, в 5 кілометрах від траси М-12 "Тернопіль - Хмельницький".
Відоме з 1565 року як Хмелища. Ймовірно, назва походить від заростей хмелю, якими колись були покриті яри навколо села.
В 1807 році була побудована перша кам'яна церква Непророчного Зачаття Пресвятої Богородиці. Перша католицька каплиця була заснована коштом польської громади села в 1899 році. Землю під будівництво виділив поміщик Розенсток. В 1931-38 роках побудовано нинішній Вознесенський костел.

The Church of the Exaltation of the Holy Cross in the village of Khmelyska was founded in 1897 as the Catholic chapel of Saint Yakub.
The temple was built by the Polish community of the village on land allocated by the landowner Rozenstok. In 1905, a parish house was built nearby. The chapel was damaged during the 1911 earthquake.
In 1931-1938, a new stone church of the Ascension was built according to the project of the architect Yan Karol Sas-Zubzhytskyi, the author of the Dominican Church in Chortkiv. Neo-Gothic influence is visible in the architecture.
In Soviet times, the church was closed and used as a warehouse.
Now the Church of the Exaltation of the Holy Cross has been revived.
