The emergence of the city is connected with the beginning of industrial development of coking coal reserves in the northwestern part of the Krasnodon geological district. Simultaneously with the establishment of the first mines, the mining village of Otamanivka arose. The decision to build a town next to the village was made in 1952, and in 1954 the first streets were laid out. In 1961, the new settlement received the status of a city and the name Molodohvardiisk. In 1965, the village of Otamanivka was added to Molodohvardiisk. In 2024, in accordance with the requirements of the law of Ukraine "On condemnation and prohibition of propaganda of Russian imperial policy in Ukraine and decolonization of toponymy", the city of Molodohvardiisk was renamed Otamanivka.
