Каплиця-усипальниця роду Свейковських у Тетієві збудована 1805 року поруч із заснованим ними ж костелом Внебовзяття (Успіння) Пресвятої Діви Марії. Перед головним входом в костел колись стояла скульптура святого Яна Непомука, тому храм також називали його іменем.
